Lost or Stolen PolyCards

Meal Plan and Campus Express Account cardholders should call 756-5939 to report a lost or stolen PolyCard immediately. Customer Service will deactivate the card. If you find your card after calling to deactivate it, you must bring it in person to the Customer Service Office to have it reactivated.

If you find someone else's lost card, please bring it to the Customer Service Office or drop it in any Campus Express Station.

Faded and Worn Out PolyCards

If your PolyCard is worn out from years of use, bring it in to the Customer Service Office for a free replacement.

Special Notes Regarding Replacements

All requests for replacement PolyCards require the following:

  • A form of picture ID (driver's license, passport, etc.)
  • A completed ID Request Form, available at the office or online
  • There is a replacement fee of $5.00 for all students, faculty, and staff.
